Public Notice - Conflict of Interest Disclosure for 2022-2023 CDBG Funding
In compliance with 24 CFR 84.42 and 570.611 and the 2017 – 2018 CDBG Application requirements, the following disclosure is posted for a 15-day public comment period through May 6, 2022.

Project Name: Roma Berry Center for Seniors, Gymnasium and Activity Center
CDBG Grant Amount: $440,299
Funding Year: 2022 – 2023
Description of Project: LIFE Senior Services, Inc. plans to use federal grant funds to assist the organization in constructing a gymnasium and activity center for seniors as part of their nine acre campus located at 4821 South 72nd East Avenue, Tulsa, OK 74145 . As a result of this federal assistance, at project completion, the organization will be obligated to operate the facility for its intended purpose for five years and report those served during that time period.
Brief Description of Potential Conflict: A LIFE Senior Services employee, Carolyn Blair, Director of Development, is spouse to Jack Blair, a City of Tulsa employee who served as Chief Operating Officer in the Office of the Mayor during the grant allocations process. A public disclosure of this relationship was filed with the City Clerk’s office on February 9, 2022 which stated that Mr. Blair will not participate or assist in any City business, matter, or deliberation involving LIFE Senior Services.
